New Procedure for Incident Reporting

It is company policy that ANY and ALL incidents (including near misses), injuries (including minor) and/or illnesses involving ANY Meadville Land Service, Inc. employee, property and/or jobsite shall be reported. Please complete the First Report of Incident as outlined below:


Near Miss:

1. Complete ALL AREAS of the First Report of Incident

2. Completed form to the PR/HR Department within 7 days.

Motor Vehicle / Heavy Equipment Accident:

1. Call 9-1-1 if necessary (i.e. injury, personal and property damage, etc. – supervisor’s discretion)

2. Call MLS

3. Complete ALL AREAS of the First Report of Incident.

4. Completed forms to the PR/HR Department within 7 days.

First Aid Only:

1. Complete ALL AREAS of the First Report of Incident.

2. Completed form to the PR/HR Department within 7 days.

Medical Treatment:

1. Call 9-1-1 when necessary

2. Call Workplace Injury Triage 1-844-334-6470

3. Call MLS.

4. Complete ALL AREAS of First Report of Incident

5. Completed form to the PR/HR Department within 7 days.

Fatality:

1. Call 9-1-1

2. Call Robin immediately.

3. Complete ALL AREAS of the First Report of Incident.

4. Completed form to the PR/HR Department within 7 days.


**The Incident Report on your Supervisor Website has been modified, please complete that immediately after an incident occurs**

**The First Report of Accident Form should be turned in with your weekly paperwork**
